Breaking bad news is a complex skill as, in addition to the verbal component, it also requires the ability to recognise and respond to the patient’s emotions, dealing with the stress that the bad news creates and yet still being able to involve the patient in any decisions and maintaining hope where there may be little. There is also the likelihood that it will be a triadic consultation with relatives present. SPIKES is a six-step protocol which has been shown to improve the confidence of clinicians who use it when breaking bad news to cancer patients:[1] :
Setting up the interview.
Assessing the patient’s Perception.
Obtaining the patient’s Invitation, as shunning information is a valid psychological coping mechanism.
Giving Knowledge and information to the patient.
Addressing the patient’s Emotions with Empathetic response.
Having a Strategy and Summarising.

Janelle Chase-Mayers has always had a love for sports, exercise and fitness. And it became a natural progression for her to make it part of her life path. While attending Barbados Community College pursuing her Associate Degree in Physical Education at the tender age of 17 in 1999, she was awarded a Div 1 scholarship to play volleyball in the states, and her awareness of health and wellness, sport and fitness grew. From travelling to play sports around the United States, Caribbean and South America, Janelle recognized the need for an athletic facility that would support the national teams and the general population. She continued with her education in the form of a Bachelor’s degree in Psychology and Masters in Marriage and Family Therapy from the University of Louisiana at Monroe. Wrapped up in those degrees are her love for sports and fitness, with over 40 credit hours of Anatomy and Physiology, weight lifting and training and countless hours as an athlete.
The beauty of sport and fitness is that it draws total strangers, communities and families together, and with that Janelle uses sport to strengthen her relationship with her 6 year old son Tj and her larger family context. In the near future, Janelle is currently enrolled in her PHD program in Creative Systemic Studies in Marriage in Family Therapy, with an emphasis on how Health and Wellness Strategies can improve familial relations and promote growth to a larger community. She is the Head Coach/Owner of CrossFit 246 and a practicing Marriage and Family Therapist. Janelle has conducted several workshops around the island and Caribbean focusing on Improving Interpersonal Skills, Motivational Interviewing, Understanding Addictions in Families, Understanding emotional intelligence through the Iceberg Theory.
