From palpitations to chest pain to leg swelling, cardiovascular complaints keep the General Practitioner, Emergency Specialists and other health professionals busy. This CME explores some common symptoms which may suggest cardiovascular disease and the differential diagnoses outside the cardiovascular system. We will focus on the approach to diagnosis and treatment. Presenters include local Cardiologists, Emergency Specialists, Psychiatrist, Internists and others. Doctors, nurses, allied health, mental health professionals & other interested persons are invited to attend. CPE credit is available.

10:00- 10:45 a.m. Palpitations: Dr. Aderemi Soyombo, Cardiologist

The differential diagnosis for palpitations is broad, ranging from anxiety disorders to life-threatening arrhythmias. Dr. Soyombo will discuss the approach to the investigation of palpitations. Red flags will be discussed.

10:45- 11:30 a.m. Syncope: Dr. Stephen Moe, Cardiologist

Fainting is always a dramatic event. Yet it can have a relatively benign cause. When should we be concerned? Dr. Moe will review the approach to the patient with syncope, from the all-important history to advanced investigations such as invasive rhythm monitoring.

11:30 a.m. to 12:00 p.m. Chest pain in the ambulatory setting – Dr. Dawn Scantlebury, Interventional Cardiologist

12:00-12:30 p.m. Chest pain the emergency setting- Dr. Michael Wharton, Emergency Physician

When patients experience chest pain, the first thing that comes to mind is a Heart Attack! Yet, there are multiple structures within the chest, abdomen and neck that can result in chest pain. On the other side of the spectrum, a heart attack may present without chest pain. How confusing can it get?! Dr. Alexander will discuss the approach to the patient with chest pain in the ambulatory setting and Dr. X in the emergency setting. We will discuss clues to a life-threatening differential diagnosis

1:30-2:00 p.m. Shortness of Breath – Anne-Marie Hassell, Intensivist/ Pulmonologist  

2.00-2.30 p.m.        Shortness of Breath: A Primer for Nurses – Tracy Davis, RN

Is it asthma? Is it heart failure? Dr. Harvey will discuss the approach to the patient with shortness of breath.

2:30-3:00 p.m. Leg Pain/ Claudication: Dr. Nina Yhap, Senior Registrar, Surgery

Barbados is known in some quarters as the amputation capital of the world. Yet this is an avoidable complication of peripheral arterial disease. Dr. Nina Yhap will guide us through the clues to considering the diagnosis of PAD, as well as its investigation and management.

3:00-3:30 p.m. Leg Swelling: Jocelyn Brookes, Vascular Surgeon

Is that swelling in your obese patient heart failure? Or is it the ever-prevalent venous disease? Dr Brooks will focus on the diagnosis and management of venous disease along with a brief discussion of the differential diagnosis.

3:30-4:00 p.m. Cardiovascular Disease & Stress:

Stress! We all experience it, and some handle it better than others. How does it impact your patient? Is that cardiovascular complaint simply a manifestation of poor stress coping skills?

Biography: Dr. Tania Whitby-Best

Dr. Whitby-Best graduated from the Faculty of Pure and Applied Sciences UWI Mona in 2000 and the Faculty of Medical Sciences UWI Mona in 2005. Following the completion of her internship at the Queen Elizabeth Hospital Barbados, she joined the department of Otolaryngology and then subsequently joined the Family Medicine, Doctor of Medicine programme, which she completed in 2014.

 She is presently Medical Officer and Head of the Student Health Clinic UWI Cave Hill Campus, and a part time tutor with the Faculty of Medical Sciences UWI Cave Hill.

Dr Whitby-Best is passionate about mental health and primary care. This interest has led to research on the topic of depression among type 2 diabetics in Barbados and she hopes to continue to make contributions to this area in the Caribbean. She is presently the honorary secretary of the Caribbean College of Family Physicians and has served on other professional committees such as Junior Doctors Association. Besides the practice of medicine, Dr Whitby-Best has a keen interest in the performing arts especially dance and has been an active dancer and choreographer during and post her university years.

Dr. Kadisha Douglin is a senior registrar at the Psychiatric Hospital. She gained her MBBS in 2008 and successfully completed her postgraduate training in psychiatry at the University of the West Indies Cave Hill campus in august 2017.

Individuals living with type 1 or type 2 diabetes are at increased risk for depression, anxiety, and eating disorder diagnoses. Among the wide-ranging conditions associated with diabetes, mental health issues are probably among the most overlooked, despite their potential to compromise self-management and increase risk for serious short- and long-term complications, which can result in blindness, amputations, stroke, cognitive decline, decreased quality of life, and premature death. This public lecture explores the intricacies of diabetes and mental health, prevention and management strategies and the resources available in Barbados to assist in the mitigation of this condition.

Pre-event activities start at 5pm and include: Free blood sugar and blood pressure checks; product displays, sampling, diabetes education, giveaways and more. Transportation will be provided to the Princess Alice & Fairchild Street Bus Terminals by the Transport Board.

All are invited to attend. No CPE credit is available for health professionals.

From syncope to abnormal heart rhythm to heart failure, cardiovascular complaints keep the General Practitioner, Emergency Specialists and other health professionals busy. This CME explores some common cardiovascular complaints and ways in which they can be correctly diagnosed, treated and prevented. The relationship between psychiatric disorders and cardiovascular health will also be explored including how the psychological and physiological effects of stress could contribute to cardiovascular disease etiology. Presenters include local Cardiologists, Emergency Specialists, Psychiatrist, Internists and others. What You Should Know About Post Menopausal Bleeding

The National Health Service looks at

Postmenopausal bleeding

Menopause is usually diagnosed in women over 45 who haven’t had a period for more than a year. Any bleeding from the vagina after this needs to be checked by a GP.

See a GP if you have postmenopausal bleeding, even if:

  • it’s only happened once
  • there’s only a small amount of blood, spotting, or pink or brown discharge
  • you don’t have any other symptoms
  • you’re not sure if it’s blood

Postmenopausal bleeding isn’t usually serious, but can be a sign of cancer. Cancer is easier to treat if it’s found early.

What happens at your GP appointment

Your GP should refer you to hospital or a special postmenopausal bleeding clinic for further tests.

If you’re over 55, you shouldn’t have to wait more than 2 weeks to see a specialist. If you’re under 55, you may have to wait longer.

What happens at your hospital or clinic appointment

A specialist, who may be a nurse, will offer you tests to help find out what’s causing the bleeding and plan any necessary treatment.

The tests may include:

  • a small device being placed in your vagina to scan for any problems (vaginal ultrasound scan)
  • an examination of your pelvis and vagina
  • a thin, telescope-like camera being passed up your vagina and into your womb to look for any problems (a hysteroscopy) and to take a tissue sample (biopsy) for testing – under local or general anaesthetic

Causes of postmenopausal bleeding

There can be several causes of postmenopausal bleeding.

The most common causes are:

  • inflammation and thinning of the vaginal lining (atrophic vaginitis) or womb lining (endometrial atrophy) – caused by lower oestrogen levels
  • cervical or womb polyps – growths that are usually non-cancerous
  • a thickened womb lining (endometrial hyperplasia) – this can be caused by hormone replacement therapy (HRT), high levels of oestrogen or being overweight, and can lead to womb cancer

Less commonly, postmenopausal bleeding is caused by cancer.

Treatment for postmenopausal bleeding

Treatment depends on what’s causing your bleeding.

Dr Nicole Durham attended The Lodge School before starting her medical training at the University of the West Indies, Mona Campus Jamaica in 2000 .During her training she attained honors in anatomy, physiology and pharmacology and completed one area of training at St. Michael’s Hospital Toronto,Canada and Queen Elizabeth Hospital ,Barbados.

In 2003/2004 she was awarded the Edmund Cohen (Undergraduate) Scholorship for academic merit in MBBS degree programme.

Upon completion of her Bachelor of Medicine;Bachelor of Surgery MBBS degree she was awarded as the valedictorian UWI Medical Student of the Year with The Arnott Cato Foundation award 2005 and Barbados Association of Medical practitioners prize for best student in surgery.

She has done many research papers and presentations at the E.R Waldrond annual symposium at The Queen Elizabeth Hospital during the start of her residency programme in the department of Obstetrics and Gynaecology from 2007 to 2009.

After a short surgical journey at King Edward Memorial Hospital in Bermuda,she then returned to Jamaica to complete her Doctorate (DM) in Obstetrics and Gynaecology at the University of the West Indies , Mona Campus in 2013. She is a Fellow of the American congress of Obstetricians and Gynecologists, Member of the international Society of Ultrasound in Obstetrics and Gynaecology, American Cellular Medicine Association and European Society of Aesthetic Gynaecology.

Dr Durham returned to Barbados in 2014 and has been working in private practice as the Medical Director of MacMedical Women’s Clinic ,The Barbados Family Planning Association and Brandford Taitt Polyclinic.

Currently she is a mother ,wife , and medical advisor to the Hey Vajayajay Show ,The Barbados Association of Endometriosis and Polycystic Ovarian Syndrome and Better Birthing in Bim charities.

Her topic at the Annual Women’s Health Medical Conference is Menopause: Things to Consider.

Guest Speaker: Simone McConnie

Simone McConnie, BSc Pod Med, MChS HPC registered (UK) is a Podiatrist trained in the UK, and has been working in Barbados since 1994.   She holds a degree in Podiatric Medicine and MBA from Durham University in 2011.  Her main clinical interest is diabetes, and wound care.

She worked at the first established Diabetic Foot Clinic at King’s College Hospital, London, UK in 1993 and has been intimately involved in various projects related to promotion, education and change in diabetes care, whilst pioneering Podiatry as a new Profession in Barbados.  She has worked extensively with the International Working Group on the Diabetic foot in association with the Rotary Club of Ledbury, UK and local Rotary clubs of Barbados and the region implementing the “step by step” programme in Barbados and across the region.  A founding member and previous Trustee at the Barbados Diabetes Foundation, she currently represents Barbados on the International Working Group on the Diabetic Foot (IWGDF) and an International working group for strengthening the Podiatry profession Internationally under the auspices of her International professional body The Society of Chiropodists and Podiatrists (SOCAP).

She is a member of the American Association of Wound Care (AAWC), The Worldwide Association of Female Professionals and The President of the Barbados Association of Podiatrists. Her topic at the Annual Women’s Health Medical Conference will be Podiatric Issues in Women.

Profile

Dr Soyombo is a Non-Invasive Cardiology Specialist. She currently works as a Consultant within the Department of Cardiovascular Services at the Queen Elizabeth Hospital and is the founder of Healthy Heart Inc, on River road, St Michael; a practice dedicated to the early prevention, diagnoses and management of heart disease.  

 

She completed her Internal Medicine training in 2006 at Cornell University/New York Presbyterian Hospital in New York. This was followed by Fellowship training in Cardiology at Georgetown University/Washington Hospital Center in Washington DC.  She then went on to practice at Heart and Vascular Associates in New Jersey where she was Director of the Echocardiography lab.   She also served as Attending Physician at St Joseph’s Regional Medical Center where she was involved in medical education, and as a Consultant at other area hospitals.
